CCOs are organizations that were founded by current developmental disability care providers to provide better services to their clients, staffed by Care Managers.

These new companies are staffed by Care Managers who have received specialized training and have worked in the field of developmental impairments for several years.

A CCO enrollment form will need to be reviewed and signed by your Medicaid Service Coordinator before you may begin participating in the CCO program of your choosing. You have the option of selecting the CCO with which your existing MSC has become linked, or you can select a different CCO in your geographic region. As part of the selection process, you will have to pick between Health Home Care Management and Basic HCBS Plan Support.

Health Home Care Management – the more robust, comprehensive care management service which provides:

  • Coordination of the developmental disability services that you and your family need or are currently receiving,
  • Comprehensive coordination of other health and behavioral health care services you need or receive.

Basic HCBS Plan – focuses on planning related only to HCBS waiver services and does not coordinate any other health or behavioral health services. With this service, your Care Manager will work with you to:

  • Develop, review and update your Life Plan (service plan),
  • Ensure that your annual, needed Level of Care Eligibility Determination (LCED) is completed.

If you are dissatisfied with the Care Management given by the CCO you have chosen, you have the option of selecting a different Care Manager within that CCO and/or changing the quality of care you are receiving. You have the option of selecting a different CCO within your region.

The term “Health Home Care Management” refers to services offered by Care Coordination Organizations, which are also known as “Care Coordination” (CCOs). There are seven CCOs in operation in the state of New York. The seven CCOs are:

Each of these seven CCOs operates in a different region of New York State, with some degree of overlap for various counties.

Advance Care Alliance operates on Long Island, New York City and the counties due north of New York City. These include the counties of:

Partners Health Plan FIDA-IDD

Partners Health Plan FIDA-IDD operates in Long Island, New York City, and the counties just north of New York City. These include the counties of:

  • Richmond (Staten Island)
  • Rockland
  • Suffolk
  • Westchester

In order to receive support and services, people with developmental disabilities must submit an application to the Office for People with Developmental Disabilities (OPWDD). In the OPWDD, the Eligibility Determination Procedure is the name given to the application and review process for OPWDD services. Only the OPWDD has the authority to determine whether or not a person satisfies all of the requirements for eligibility for OPWDD-funded services.

If your child is judged to be eligible, you will need to pick a Care Coordination Organization (CCO) if you do not already have one in order to receive care coordination services and a care manager. If your kid does not have Medicaid coverage, the CCO can assist you in obtaining coverage for Medicaid funded services for special needs. The majority of OPWDD services and assistance are covered by Medicaid. Please keep in mind that you can also contact a CCO and collaborate with their intake team to complete the eligibility assessment process from the start.
